Job Description
The Supportive Housing Case Manager (Ages 18–25) supports young adults in securing and keeping stable housing while building life skills and independence. Using a positive, youth-centered approach, you’ll carry a small caseload (12–18 clients), offer flexible one-on-one guidance, and connect them to education, jobs, and community resources. This is a rewarding role for anyone who enjoys cheering young people on as they step confidently into adulthood.
Responsibilities:
Conduct intake assessments, develop individualized housing stability/service plans, and update plans quarterly or as needs change.
Assist clients in securing and maintaining permanent housing (lease signing, landlord mediation, rental arrears negotiation, unit inspections, etc.).
Coordinate medical, mental health, substance use treatment, benefits enrollment (SSI/SSDI, SNAP, Medicaid), employment, and other supportive services.<...
