The Regional Team Facilitator supports clinical and operational functions across the region, providing collaborative assistance to both field clinicians and office-based teams. This role helps ensure the accuracy, integrity, and compliance of electronic medical records (EMRs) and clinical documentation. The Team Facilitator promotes efficient workflows, supports regulatory standards, and enhances continuity of patient care through coordinated oversight and operational support. Oversee and coordinate the Resumption of Care (ROC) process, including tracking patients on transfer and ensuring timely communication to all departments regarding transitions back home. Monitor and manage coding queries and ensure timely, accurate responses to meet coding team requirements.
