Job Description
Title: Physical Therapist (PT) Home Health / Hospice
Location: Dublin, OH
Duration: 13 weeks
Shift: Day shift, 5x8 hours (8:00 AM 5:00 PM)
Job Responsibilities
-
Provide physical therapy services to geriatric patients in a home health/hospice setting

Manage an average caseload of 5 7 patients per day

Complete Start of Care (SOC) visits when required

Perform timely, accurate clinical documentation and maintain clear communication

Collaborate with interdisciplinary care teams

Participate in on-call rotation (approximately every 6th weekend)

Travel within an assigned territory up to a 60-mile radius
