Transplant Care Navigator - On-Call Multidisciplinary Team

SingHealth is seeking a Transplant Coordinator responsible for identifying potential organ donors and providing essential support to patients throughout their transplant journeys. You will collaborate with a multidisciplinary team and assist in organ donation initiatives to promote awareness.

The ideal candidate should hold a degree in Nursing, Psychology, or Life/Health Sciences, possess excellent interpersonal skills, and be willing to perform rotational on-call duties. This role includes training to support various transplant programs within the organization.
