- Completion of FACIS Questionnaire if any disciplinary actions exist.
- Must have current license and primary source verification within 30 days prior to start.
- Must complete Ascension Core Competencies with minimum 80% passing score.
- Completion of Ascension EKG Exam with at least 80% passing score.
- Completion of Ascension Skills Checklist within one year of start; updated annually for specialty.
