Travel LPN Opportunity:

• Implement, develop, and document the plan of care in collaboration with other medical professionals to ensure quality and continuity of care
  


• Provide direct patient care services to hospice patients and supportive care to the patient's family
  


• Act as a primary liaison between the physician, patient/family, and hospital team
  

Travel LPN Qualifications:
