- Massachusetts nursing license with primary source verification within 30 days of start date
- Fit Test (N95) within 365 days of start date and updated annually
- Respirator Medical Clearance Attestation required if fit test is required
- MGB Medication Exam with 80% passing within 365 days
- MMR vaccination or proof of immunity required
- TB test documentation within 3 months of start date and updated if >6 month break
- Health Statement signed by MD/DO/NP/PA within 365 days of start date and updated if >6 month break
- MGB Criminal Background Check within 45 days prior to start date and update...
