- Extension for Leonard Wisniewski for 12 hour nightshift, 36 hours/week
- Job Requirements
- Must have valid State License Verification completed within 30 days prior to start
- BLS certification required and must not expire while on assignment
- Centralus Skills Checklist for the unit the traveler is working on, updated annually
- Centralus Core Competency Exam with a score of 80% or higher, updated annually
- Fit Test must be completed prior to start and within the last 12 months
- Hepatitis B vaccination or positive antibody titer or declination form required
- TB Test with valid results within required timeframes, annual requirement
- MMR vaccination or positive IgG titer required
- Varicella vaccination or positive IgG titer required
