Overview


The Vice President of Care Transitions, Home Health is responsible for the strategic leadership, development, and execution of the organization’s Care Transitions program across home health operations. This role drives operational growth through hospital and senior living partnerships, embedded transition resources, and referral optimization strategies. The VP of Care Transitions defines and operationalizes the care transition model, ensuring scalable processes that support increased episodic admissions, institutional growth, and market expansion. This leader works cross-functionally with Business Development, Operations, Clinical, and Home Health Sales teams to strengthen referral pathways, enhance patient access, and improve care coordination and operational performance.
