Within the context of a client‑centred care model and in accordance with the established vision and values of the organization, the Wellness Support Worker works as a member of an interdisciplinary community health team, providing day‑to‑day physical, emotional and social supports, advocacy, life‑skills assistance, information, resources and demonstrations to designated population groups in the community. Reports back to the interdisciplinary team on clients’ progress and condition, establishes working relationships with various agencies, and co‑facilitates support groups, cultural activities and public education.
